Are you an experienced critical care professional ready to take the next step in your clinical career? Join our forward-thinking Critical Care Outreach Team (CCOT) as we transition into advanced clinical practice, enhancing the response to deteriorating patients across the hospital.
We are seeking two dynamic and motivated individuals to join our expanding team. As part of the CCOT, you will work autonomously to support ward-based teams in managing acutely unwell patients, playing a vital role in improving patient safety and clinical outcomes beyond the critical care setting.
About The Role
- Full-time substantive post (minimum 30 hours/week)
- Participation in an extended hours and weekend rota
- City-wide working across Sheffield Teaching Hospitals sites
- Supported enrolment onto a Master’s-level Trainee Advanced Clinical Practitioner (ACP) programme
- Dedicated study time for academic work alongside clinical duties

- Completion of CC3N Steps competencies 1, 2, and 3 or equivalent.

As a Trainee ACCP in CCOT, you will work autonomously to deliver high-quality, individualised care to acutely ill and deteriorating patients outside of the critical care unit.
Clinical Responsibilities
- Perform comprehensive A–E assessments, create and implement care plans to prevent deterioration.
- Monitor interventions, adjusting as needed for optimal patient outcomes.
- Provide expert advice and clinical support regarding critical care.
- Participate in the hospital cardiac arrest team as a clinical leader.
- Support post-critical care patients, offering both physical and psychological follow-up and appropriate referrals.
Education And Training
- Deliver trust-wide education, including bedside teaching and Deteriorating Patient Study Days.
- Promote best practices and contribute to ward staff development.
Service Development And Collaboration
- Represent and promote CCOT across the Trust, building strong relationships with ward teams.
- Collaborate with multidisciplinary teams to ensure cohesive patient care.
- Engage in service improvement and development efforts within CCOT.

Working Hours: The role currently covers 08:00–18:00, seven days a week. Be prepared for future 24/7 coverage and cross-site work as the service expands.
