We are looking for an adults Experienced Social Worker to join us in the Community Future Options (CFO) team. We are a long-term adult social care team who predominantly work with people aged 18 to 65 with eligible care and support needs under the Care Act 2014. Our work involves the completion of Care Act assessments, support planning, Mental Capacity Act assessments, and safeguarding enquiries.

Apply a Strengths-Based Approach to support individuals to live fulfilling lives at home, connected to their community and local resources. Provide care and support where needed, promoting independence, choice, and recovery. Engage in effective partnership working with other colleagues and agencies to ensure a holistic approach to support.Assessment and Planning
Assess needs and wellbeing under the Care Act 2014 to determine eligibility. Produce written assessments evidencing eligibility and support needs to inform professional decision-making. Co-produce support plans with individuals and undertake risk assessments.Information and Advice
Provide information and support to facilitate wellbeing for people and carers. Knowledge of referral and signposting processes for Housing, Welfare Benefits, and community support services.Collaboration
Work with other professionals and the voluntary sector to support individuals and carers. Lead on complex safeguarding cases, VARMM, and risk reduction, informed by relevant legislation and policy.Legal and Compliance
Work within the principles of the Mental Capacity Act 2005, assessing capacity and acting in best interests. Attend legal forums and produce complex reports (e.g., observation reports, witness statements). Utilise IT systems to record actions in line with GDPR.Leadership and Development
Support, mentor, and supervise social workers through ASYE and CPD pathways. Lead responsibility for mentoring student social workers and supporting social work peers. Contribute to practice development and share areas of expertise to promote a learning culture within the team.Resource Management
