We are seeking enthusiastic Home Care Support Workers to support children with complex biopsychosocial needs related to excessive weight. You will help families make positive lifestyle changes while working as part of a multidisciplinary team—including specialist nurses, consultants, dietitians and psychologists—providing support in both clinic and community settings.

About You

You are confident working independently and as part of a multidisciplinary team alongside consultants, specialist nurses, psychologists and dietitians. You bring excellent communication skills, GCSEs in English and Maths (A-C), and an NVQ Level 3 in Health and Social Care or equivalent. A full UK driving licence is required.

Main duties of the job

  • Support children, young people and families within the Specialist Paediatric CEW Service across Sheffield, Rotherham, Doncaster and Barnsley.
  • Provide healthy eating guidance, health promotion and lifestyle advice.
  • Deliver support in clinics, during home visits and within community settings.
  • Work autonomously, managing your own workload while contributing to a multidisciplinary team.
  • Build effective relationships with families to encourage positive lifestyle changes.

Qualifications

  • GCSE A-C English or Level 2 equivalent.
  • GCSE A-C Maths or Level 2 equivalent.
  • Vocational Award Level 2 in Health.
  • To have or be committed to undertaking & completing a Vocational Award Level 3 in Health or Health & Social Care or diploma.

Experience

  • Experience in a Care or Health & Social Care environment within a hospital or community/home setting.
  • Worked in a customer-facing environment.

Desirable Experience

  • Worked with children and young people.
  • Experience of working with vulnerable groups.

Knowledge and Skills

  • Knowledge of Health & Safety and maintaining a safe environment.
  • Knowledge of Safeguarding children or young adults.
  • Basic IT skills which include competency at using Word and email and commitment to learning advanced IT skills.
  • Ability to work with children, young people, their families and carers.
  • Ability to work in a multidisciplinary team.
  • Ability to show initiative and take on own responsibility and accountability and work independently.
  • Ability to prioritise workload and manage own time.
  • Ability to deal with conflicting and challenging difficult situations.
  • Calmness and resilience.
  • A valid driving licence and access to a car with insurance.

Desirable Knowledge and Skills

  • Compliance of Policies and Procedures in a regulated Health Care Provider.
  • Advanced IT skills, e.g., Level 2 IT qualification.
  • Previously worked in a Health / Social Care multidisciplinary team setting and contributed in team meetings.
  • Experience of having a caseload or key worker role.
  • Knowledge of child development.
  • Knowledge of healthy nutrition and lifestyle.
  • Evidence of safeguarding training/experience.
