The role of Senior Palliative Care Support Worker requires independent working in the community; applicants should have excellent communication skills and IT skills. You must be adaptable, reliable and highly organised to prioritise your workload effectively. This is a role where no two days are the same and you have the chance to improve the experience of patients and families when life is at its most difficult for them.
Main Duties
- Provide holistic palliative care in the last 4 weeks of life.
- Offer emotional, physical and psychological support to patients, families and carers.
- Work as part of the Hospice at Home team providing care packages in patients’ own homes.
- Variable shift pattern over 7 days including some night shifts.
- Autonomous planned care for individual patients with support of registered nurses.
- Model hospice values and contribute to a positive team culture.
- Promote safety and well-being of patients and families by following policies and procedures and working with a multidisciplinary team.
Job Responsibilities
- Provide nursing care for patients at the end of life.
- Monitor patient condition and record findings accurately verbally and in writing.
- Recognise changes in patient condition and take appropriate action.
- Identify non-clinical concerns (safeguarding, environmental risk, psychosocial challenges) and communicate to relevant professionals.
- Effectively manage volatile or unpredictable situations and call for help as appropriate for lone working, including use of a lone worker device.
- Support patients and carers to manage loss and change, including bereavement support and advice.
- Maintain a safe working environment for patients, carers and staff by following policy and procedural guidelines and attending statutory and mandatory training.
- Work within the limits of own competence and responsibility and ask for help as necessary.
Person Specification
- Essential – Qualifications: Nationally recognised qualification/apprenticeship in health & social care at Level 2 or above or equivalent experience; willingness to undertake appropriate training.
- Desirable – Qualifications: Nationally recognised qualification/apprenticeship in health & social care or palliative care at Level 3 or above.
- Essential – Experience: Willingness to develop palliative care knowledge and skills; experience of working in a health or social care setting; able to work as a member of a team; basic IT skills.
- Desirable – Experience: Good numeracy and literacy skills; experience of working in patients’ homes; experience with patients at end of life; experience of working with multiple teams; experience working independently with support of a registered nurse.
- Essential – Additional Criteria: Committed to providing high quality patient care; excellent communication skills; able to adapt quickly; ability to support people in highly distressed state.
- Desirable – Additional Criteria: Able to complete documentation accurately; aware of healthcare policies and procedures; strong IT skills; experience of using EMIS.
- Essential – Core: Understanding and empathy for hospice work; full driving licence and vehicle access; compassionate and trustworthy; good relationships with colleagues, patients and families; flexible and adaptable to change; calm, caring patient manner; enthusiasm and commitment to working with patients at end of life and families.
