We have an exciting opportunity for Band 6 Palliative Nurse Specialists to join our Community Nursing teams. We are a supportive and dynamic team delivering outstanding palliative care to people in our community. You will need knowledge and experience of palliative care, confident decision‑making skills and a passion to continuously improve and develop. This role is primarily advisory, with minimal hands‑on nursing care.
The Community Nurse Specialist is an expert in ensuring high‑quality palliative care is delivered to patients and their families. They possess specialist knowledge and skills in physical symptom control, social, emotional and spiritual care. They work both independently and as part of the wider healthcare team.
Main Duties:
- Conduct holistic assessment (remote and face‑to‑face) to identify patient and family needs.
- Deliver specialised palliative advice, support and education to patients, relatives, carers and external professionals, escalating to the hospice multidisciplinary team as appropriate.
- Assess routine clinical referrals to St Peters Hospice, determining urgency and needed care.
- Provide clinical governance support, including non‑medical prescribing and use of EMIS database when required.
- Act as a resource for internal and external professionals on specialist palliative and end‑of‑life care issues.
- Support and coach carers at home in managing a deteriorating relative.
- Recognise and manage palliative care emergencies, providing safe transitions to other care providers.
- Participate in MDT meetings, deputise for the team leader when necessary, and contribute to service improvement and quality initiatives.
- Maintain professional documentation and communication using electronic patient records.

Person Specification:
Essential Qualifications:
- Current NMC registration
- Relevant degree level qualification (or equivalent)
- Communication skills training, or willingness to undertake
- Valid UK driving licence
Desirable Qualifications:
- Mentoring training
- Experience of teaching
- Non‑medical prescribing, or willingness to undertake
- Physical assessment and clinical reasoning, or willingness to undertake
Essential Experience:
- Minimum 2 years post registration
- Recent significant palliative care experience
- Knowledge of clinical governance and evidence‑based practice
- Knowledge of safeguarding individuals at risk
Desirable Experience:
- Knowledge of community palliative care issues
- Knowledge of statutory and voluntary agencies and organisational structure
- Experience of advance care planning conversations
- Experience of audit processes
- Experience of using EMIS database
- Experience of symptom management for malignant and non‑malignant disease
