Hampshire County Council is seeking a Community Reablement Assistant (Level 2) to support adults over 18 in Hampshire, helping them regain independence and remain at home. You’ll conduct initial home visits, carry out reablement interventions and work with a multi-disciplinary team including Occupational Therapists and Health colleagues.
You should have social or health care experience, a patient, empathetic approach, and IT skills (Microsoft 365).
