Shared Lives South West is an award-winning charity that provides long-term and short break care and support services throughout Devon, Cornwall, and Somerset. We’re now looking for a passionate individual to join us as a Shared Lives Coordinator on a part-time, permanent basis, working 30 hours per week. Your days worked must include Wednesday, Thursday, and Friday.

Responsibilities Summary

As a Shared Lives Coordinator, you will recruit, assess and match Shared Lives Carers with people seeking care and support, ensuring high-quality and well-monitored arrangements. Working closely with carers, supported individuals and partner agencies, you will support the development of positive, person-centred relationships, ensuring arrangements are safe, effective and aligned with the Shared Lives ethos. You will bring experience of working with people who have care and support needs, alongside strong communication skills and a compassionate, organised approach to supporting both carers and individuals.

A Bit More About The Role

This is a varied and rewarding role where you will work closely with Shared Lives Carers and the people they support, helping to create and maintain arrangements that are safe, supportive and built around individual needs. Undertaking comprehensive assessments of prospective Shared Lives Carers, you will support their journey through the approval process, ensuring they meet the requirements of the scheme and relevant regulatory frameworks. You will also support the introductory process, helping to ensure successful matches between carers and supported individuals, and will provide ongoing support through regular telephone contact and home visits. Alongside this, you will manage a caseload of Shared Lives Carers, offering guidance and responding to any challenges that arise within arrangements, including more complex situations and those relating to safeguarding.

Additionally, you will:

  • Liaise with funding and benefits teams regarding financial aspects of the service
  • Build and maintain positive relationships with key personnel and partner agencies
  • Support training events, local meetings and social opportunities for carers and supported individuals
  • Contribute to service promotion through calls, visits, presentations and networking

Qualifications Who Are We Looking For?

To be considered as a Shared Lives Coordinator, you will need:

  • At least two years’ experience working within a regulated care or commissioning environment
  • Knowledge and experience of person-centred approaches when working with people who receive support
  • Sound knowledge of legislation, policy and practice relevant to adults with care and support needs (e.g. Care Quality Commission, Safeguarding Adults, Mental Capacity Act)
  • An understanding or experience of the Shared Lives model
  • Knowledge of local authority social care processes
  • A qualification at Level 3 or above in Health and Social Care (or equivalent)
  • GCSEs at Grade C/Level 4 or above in Maths and English, or equivalent
  • A full, valid driving licence and the ability to travel according to the needs of the role
