Sevenfields Primary Care Network (PCN) is seeking a self-motivated and experienced Advanced Nurse Practitioner to join our multidisciplinary team under the Additional Roles Reimbursement Scheme (ARRS). The role is to provide an acute home visiting service, assessing housebound patients presenting with acute illness and injuries, as well as performing long-term condition reviews for this subset of patients as required by the four practices within the PCN, serving a combined population of approximately 41,000 patients.

Working closely with GPs, nurses, and the wider primary care team, you will help deliver high-quality, patient-centered care to housebound and vulnerable individuals, supporting continuity of care and proactive management of long-term conditions. You will be accountable to Sevenfields PCN and will collaborate across practices to ensure service flexibility, resilience, and effective cross-cover. This role plays a vital part in supporting our GP practices by carrying out home visits, helping to reduce hospital admissions, and enhancing patient outcomes through the delivery of timely and effective healthcare in the home setting.

Main duties of the job

  • Provide high-quality, evidence-based care to patients as an Advanced Nurse Practitioner in General Practice.
  • Manage a clinical caseload, assessing and addressing patients needs within a primary care setting.
  • Work closely with GP practices and the multidisciplinary team (MDT) within the PCN, ensuring patients receive coordinated, high-standard care.
  • Contribute to the development and implementation of service improvements to enhance care delivery across the PCN.
  • Collaborate with colleagues across health, social care, and the third sector to deliver patient-centred, integrated care.

Job responsibilities

  • Assess, diagnose, plan, and evaluate care for housebound patients with undifferentiated, acute, and chronic conditions.
  • Conduct comprehensive assessments, including medical histories, physical examinations, considering physiological, and social factors.
  • Manage complex, urgent, and emergency presentations, initiating appropriate emergency care when required.
  • Proactively identify, diagnose, and manage long-term conditions, including completion of annual QoF reviews and locally commissioned service requirements.
  • Prescribe, review, and optimise medicines within scope of practice and in line with national and local protocols.
  • Deliver clinical interventions including wound care, phlebotomy, and vaccination and immunisation programmes.
  • Develop and implement individualised management plans, referring to or seeking advice from GPs and clinical leads as appropriate.
  • Provide patient education, health promotion, and preventative care to support self-management and long-term wellbeing.
  • Work collaboratively with GPs, nurses, and the wider multidisciplinary team to ensure coordinated, holistic care across home and clinical settings.
  • Maintain accurate and timely clinical documentation on practice systems (e.g. EMIS) and contribute to reducing hospital admissions and improving patient outcomes.

Person Specification

Qualifications

  • Registered General Nurse with the Nursing & Midwifery Council.
  • Masters Degree OR Postgraduate diploma or degree for Advanced Practice Qualification OR RCN accreditation.
  • Meets NMC revalidation requirements.
  • Meets the standards for registered ANP working at an advanced level.
  • Valid UK driving licence with no more than 4 penalty points.
  • No outstanding investigations or complaints.
  • Qualified triage nurse.
  • Experience in end-of-life care and planning.

Experience

  • Minimum of 5 years post registration experience.
  • Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge and on-going competence in all aspects of the ANP role.
  • Experience in management long term conditions e.g. asthma, COPD, diabetes, CHD.
  • Evidence of working autonomously and as part of a team.
  • Proven ability to evaluate the safety and effectiveness of their own clinical practice.

  • Proficient in IT systems, including EMIS Web, Docman, and Microsoft Windows.
  • Strong telecommunication skills with a professional and empathetic phone manner.
  • Maintains strict confidentiality and patient data protection at all times.
  • Aware of policy developments relevant to primary care, including General Practice, GMS/PMS contracts, Clinical Governance, QoF, and PCN DES contracts.
  • Knowledge of national standards and guidelines that inform clinical practice, including NICE guidelines and National Service Frameworks.
  • Knowledge of public health issues.
  • Able to identify determinants on health in the area.
