To take responsibility for ensuring the criteria for CPD is met in order to maintain nursing registration and also for professional and organisational knowledge in accordance with PREPP. To practice according to Bromley Healthcare Policies and Procedures. Demonstrate active interest in at least one key area of community nursing. To act in accordance with nurse prescribing guidelines if applicable. To actively participate in own appraisal and to access clinical supervision process as agreed. To provide integrated patient care with other members of the multi-disciplinary team. To actively participate in team meetings and disseminate information.

Main duties of the job

• Provide high quality and skilled nursing/rehabilitation care and support to patients, carers and their families of a defined caseload.
• Liaise with community teams, GP surgeries, statutory and voluntary agencies in the provision of care to patients and their families/carers to facilitate a safe discharge.
• Undertake holistic assessments of the patient, using appropriate documentation according to Bromley Healthcare's Policies and Procedures; this may include falls, continence, wound care and outcome measure assessments.
• Participate in multi-disciplinary activities such as case conferences, joint assessments, goal setting, team meetings and discharge planning.
• Plan, implement and evaluate evidence-based programmes of care.
• Ensure that all electronic and manual data is collected/recorded in an accurate and timely manner.
• Work and maintain community nursing standards as agreed by Bromley Healthcare and participate in education and peer audit.

Safeguarding

The post holder is responsible to safeguard children, young people and adults at risk. All staff must be familiar with and adhere to Bromley Healthcare's child/adult safeguarding procedures and guidelines, in conjunction with the multi-agency policies and procedures of the relevant boroughs Safeguarding Children Partnership and Safeguarding Adults Board. Staff are required to attend child/adult safeguarding training relevant to their position and required for their role.
