Position: Social Worker INT - Burnage Health Centre
The Social Worker will be working within the Integrated Neighbourhood Teams, collaborating closely with colleagues in community health to deliver tailored services to local residents. Key Responsibilities of the Social Worker:
- Provide comprehensive social work assessments and support to clients within Burnage
- Work collaboratively with multidisciplinary teams to develop and implement care plans

- Work flexibly to meet the evolving needs of the community
- Maintain accurate and up-to-date case records in compliance with company policies

Candidate Requirements:
- Qualified Social Worker with relevant HCPC registration
- Minimum of 3 years’ experience in a social work role
- Experience working within community health or similar settings
- Gaps in employment will be checked, and references will be required
- Ability to work independently and as part of a team
- Strong communication and organisational skills
