About the Role
As an Experienced Social Worker (Adults) – Grade I *, you will provide high-quality social work services to diverse adult clients, ensuring person-centred approaches are at the core of your practice. This contract role offers a dynamic environment where your skills in assessment, planning, and safeguarding will directly impact individuals' lives. You will carry a demanding but rewarding caseload, working with individuals, families, and partner agencies to foster independence and resilience.
Key Responsibilities for the Experienced Social Worker (Adults) – Grade I *
- Conduct comprehensive strength-based assessments of need, risk, and eligibility under the Care Act 2014 and other relevant legislation
- Develop, implement, monitor, and review personalised support plans to promote independence and wellbeing
- Complete Mental Capacity Act assessments and contribute to multidisciplinary teams in Continuing Health Care and transition planning
- Manage complex caseloads involving individuals with multiple needs
- Lead safeguarding enquiries under Section 42 of the Care Act, identifying and managing risks while respecting individuals’ rights
- Support individuals through life changes or crisis situations, advocating to ensure their voices are heard
- Build effective relationships with individuals, carers, and partner agencies to facilitate positive outcomes
- Participate in multi-agency meetings and contribute your expert social work knowledge

Candidate Requirements
- Full Driving Licence and access to a vehicle
- Willingness to travel inside and outside the council area
- Proven experience as a Social Worker
