Role: Care Navigator
Contract: Flexible on going hours (part-time/full-time)
Salary: £17 per hour (ltd)
Location: Services across Liverpool and Sefton
Job Summary
Contract-based opportunity within primary care. Previous experience as a Care Navigator or Receptionist within a GP practice, health centre, or primary care setting. Experience using EMIS is desirable. Excellent communication and interpersonal skills. Strong organisational and administrative abilities. A patient-focused approach with the ability to handle enquiries professionally. Ability to work effectively with healthcare professionals and administrative teams
- Provide administrative support to GP practices and Primary Care Hubs.
- Support patient enquiries and help patients navigate healthcare services.
- Previous experience as a Receptionist or Care Navigator within a GP practice or health centre is preferred.
- Handle patient enquiries and provide information about healthcare services and procedures.
- Work closely with GPs and other healthcare professionals to support effective patient care.
- Maintain accurate patient records and databases using the EMIS clinical system.
- Carry out patient navigation duties, directing patients to appropriate services and support.
- Help ensure a smooth and efficient patient experience.
- Support the day-to-day administrative operations of primary care services.
