Position: IDT Social Worker
As an IDT Social Worker , you will be responsible for supporting local and out-of-area discharge cases, ensuring safe and timely discharges while maintaining high standards of safeguarding and decision-making. This contract role lasts for two weeks, providing a focused and rewarding temporary assignment.
Candidate Responsibilities for an IDT Social Worker
- Conducting safeguarding assessments and ensuring adult safeguarding protocols are followed
- Supporting discharge planning for complex cases, including safeguarding adults in accordance with policies
- Applying knowledge of D2A pathways and processes to navigate hospital discharge procedures effectively
- Making confident, well-informed decisions in complex situations, using MCA and best interest principles
- Collaborating with multidisciplinary teams to promote strengths-based, home-first practice
- Maintaining accurate and up-to-date documentation within the hospital setting
- Contributing to a positive team environment while working predominantly in an office-based setting, with some hybrid working arrangements
