Key Responsibilities of the Hospital Adults Social Worker
As a Hospital Adults Social Worker , your role is vital in ensuring patients receive appropriate care and support during their hospital stay and transition back into the community. Your responsibilities will include:
- Conducting thorough assessments of adult patients’ social needs
- Developing and implementing care plans in collaboration with healthcare teams
- Supporting patients and families through complex decision-making processes
- Coordinating with multidisciplinary teams to ensure smooth patient discharges
- Maintaining accurate records and documentation in line with hospital policies
- Adhering to safeguarding and confidentiality regulations at all times
Candidate Requirements for the Hospital Adults Social Worker
We are looking for a qualified and experienced Hospital Adults Social Worker who:
- Holds relevant social work qualifications and registration
- Has a minimum of 3 years’ experience working in adult social care, preferably within hospital or community settings
- Demonstrates excellent communication and interpersonal skills
- Is capable of working independently in a busy hospital environment
- Has a clear understanding of safeguarding procedures and care planning
- Is available for the full 2-week contract, working office hours from Monday to Friday, 08:00 to 16:00
