About the Role – Dedicated LD Review Social Worker
As a Dedicated LD Review Social Worker , your primary responsibility will be to review cases of adults with learning disabilities, ensuring their needs are met through comprehensive assessments. You will work on a 6-month project, providing expert support and guidance to clients, and maintaining high standards of care. The successful candidate will be expected to handle a caseload efficiently, utilizing technology tools such as Liquid Logic, and adhere to all regulatory and organizational standards.
Candidate Responsibilities – Dedicated LD Review Social Worker
- Conduct detailed reviews of adults with learning disabilities
- Liaise effectively with multi-disciplinary teams, families, and external agencies
- Ensure all reviews and assessments are completed accurately and on time
- Maintain comprehensive case records using Liquid Logic or similar case management systems
- Follow up on care plans, safeguarding, and risk assessments
- Stay updated with relevant legislation and best practices
- Ensure compliance with internal policies and external regulations
- Support the well-being and independence of clients within legal and ethical frameworks
Candidate Requirements – Dedicated LD Review Social Worker
- Qualified Social Worker (completed relevant accredited training)
- Minimum 3 years' experience working with adults with learning disabilities
- Demonstrable proficiency with technology, preferably Liquid Logic
- Strong assessment and review skills
- Ability to work efficiently, managing caseloads effectively
- Not suitable for newly qualified or ASYE candidates
- Must have a 3-year reference and clear employment history
