Rate: £32 per hour Umbrella (£24 PAYE)
Contract: Temporary (with strong extension potential)
Hours: Monday - Friday, 9:00 - 17:00
Working Pattern: Hybrid (3 days office / 2 days home)
Service Care Solutions are currently recruiting for a Social Worker to join the Adults Learning Disabilities Community Care Team within the London Borough of Merton.
Key Responsibilities:
- Undertake the full Care Management process in line with the Care Act and statutory guidance
- Manage a caseload as allocated by the Team Manager, maintaining accurate and up-to-date records (paper and IT systems)
- Complete risk assessments in line with Merton Care Management Procedures
- Deliver duties under relevant legislation including the Mental Capacity Act and Deprivation of Liberty Safeguards (DoLS)
- Support safeguarding enquiries and promote Making Safeguarding Personal
- Commission care packages, ensuring value for money and quality service delivery
- Maintain accurate records on CareFirst and other systems
- Attend meetings, court hearings, and tribunals as required
- Engage in joint working with partner agencies and stakeholders
- Contribute to service development, data reporting, and continuous improvement
Requirements:
- Qualified Social Worker with relevant registration
- Experience working within Adult Social Care, ideally Learning Disabilities
