Community Psychiatric Nurse (CPN) - Dementia | Location: Telford
Contract: Full Time, NHS Contract • Hours: Monday to Friday, 9:00am - 5:00pm
Service Care Solutions are currently recruiting for an experienced Community Psychiatric Nurse (CPN) with dementia experience to join an NHS community mental health team based in Telford. This is an excellent opportunity to work within a supportive MDT while making a meaningful impact on patient care in the community.
Pay Details: £26 per hour (Umbrella) • £20.68 per hour (PAYE, excluding holiday pay)
Responsibilities:
- Conduct comprehensive mental health and cognitive assessments
- Complete risk assessments and ongoing risk management
- Develop, implement, and review person centred care plans
- Provide specialist support to service users with dementia and complex mental health needs
- Deliver crisis intervention and support to service users and their families
- Administer and monitor medication and treatment adherence
- Work collaboratively with multidisciplinary teams, including GPs, social workers, and allied health professionals
- Maintain accurate, up-to-date clinical records
- Support discharge planning and community integration
Requirements:
- NMC registration as a Mental Health Nurse (RMN)
- Previous experience working as a Community Psychiatric Nurse
- Experience supporting dementia patients
- Ability to manage a caseload independently
- Strong communication and organisational skills
