We are looking for a committed and compassionate Care Coordinator to join the Integrated Learning Disabilities Service. This is a rewarding opportunity to support adults with learning disabilities and their carers by delivering high-quality assessments, care management, and coordinated support that promotes independence, wellbeing, and person-centred outcomes.

You will be part of a supportive multidisciplinary environment, working alongside social workers, health professionals, and partner agencies to ensure people receive the right support at the right time.

Responsibilities:
  • Undertake initial needs assessments within your specialist area
  • Develop, manage, monitor and review care and support plans
  • Work closely with service users, carers, and partner agencies
  • Coordinate services and placements using available community and independent resources
  • Ensure care packages remain appropriate, effective, and value for money
  • Maintain accurate and timely records using council IT and case management systems
  • Work within financial regulations, safeguarding duties, and council policies
  • Participate in supervision and performance reviews with your line manager

  • Experience or strong ability in assessment and care planning
  • Understanding of the care management process
