Service Care Solutions are recruiting for an experienced Adult Social Worker to join Thurrock Council’s Preparing for Adulthood Team. This is an agency role, suitable for a qualified Social Worker with experience supporting young people transitioning from Children’s to Adult Social Care.

You will work with young adults with learning disabilities and impairments, supporting them through person-centred, strength-based assessments and care planning in line with the Care Act 2014. You will be joining a stable, award-winning and innovative service that promotes professional development and collaborative working.

Key Responsibilities
  • Complete strength-based assessments and reviews in line with the Care Act 2014
  • Work directly with young adults and families to identify needs, wishes, and outcomes
  • Support individuals experiencing crisis or at risk of crisis
  • Develop outcome-focused care plans and commissioning plans
  • Undertake Mental Capacity Assessments in line with the Mental Capacity Act
  • Complete Continuing Health Care (CHC) Checklists
  • Identify safeguarding concerns and follow inter-agency safeguarding procedures
  • Produce high-quality court reports and written reports within timescales
  • Work collaboratively with Children’s Services, Health, Education, and voluntary organisations
  • Maintain accurate and timely case records
  • Attend supervision, training, and contribute to service development

Requirements
  • Recognised Social Work qualification (Degree / DipSW)
  • Social Work England registration
  • Experience working with young people transitioning to Adult Services
  • Strong knowledge of the Care Act 2014, MCA, and safeguarding
  • Excellent communication and report-writing skills
