Somerset is seeking an experienced professional to join the team on an agency CHC Project. This role is ideal for someone with extensive expertise in NHS Continuing Healthcare (CHC) and a strong understanding of the Care Act 2014, particularly Section 22 and the lawful limits of local authority provision.

The Role

As a core member of the multi-disciplinary team (MDT), you will represent the local authority perspective, contributing to robust and lawful decision-making around CHC eligibility and funding arrangements.

Key Responsibilities

  • Completing CHC assessments and Decision Support Tools (DSTs) across a range of settings
  • Applying in-depth knowledge of CHC eligibility criteria
  • Representing and advocating the local authority position within MDT meetings
  • Constructively challenging decisions to support defensible outcomes
  • Exploring and advising on joint funding arrangements where appropriate

Essential Requirements

  • Extensive experience in NHS Continuing Healthcare (CHC)
  • Proven expertise completing DSTs and CHC assessments
  • Strong working knowledge of the Care Act 2014 – Section 22
  • Experience working within multi-disciplinary teams
  • Ability to provide professional challenge and support sound decision-making
