Service Care Solutions are currently recruiting for an experienced Practice Nurse to support a GP Surgery on a locum basis. This opportunity is ideally suited to a nurse seeking full-time hours, although flexible availability across Monday to Friday will be considered.

Location: Hailsham BN27

Contract: Long-Term Locum

Hours: Up to 37.5 hours per week | Part-time considered

Pay: £33 LTD per hour / £39 PAYE inclusive per hour

Key Responsibilities:

  • Deliver high-quality nursing care within a GP surgery environment
  • Lead and support Diabetes reviews, monitoring, education, and treatment planning
  • Undertake chronic disease management clinics and support long-term condition pathways
  • Complete patient assessments, care planning, and clinical interventions in line with practice protocols
  • Provide health promotion, preventative care, and self-management advice
  • Administer vaccinations, routine treatments, and support medication reviews where appropriate
  • Maintain accurate clinical records and documentation using the practice clinical system
  • Work collaboratively with GPs, ANPs, HCAs, and wider multidisciplinary teams to support continuity of care

Requirements:

  • Registered Nurse with valid NMC registration
  • Previous Practice Nurse / Primary Care experience essential
  • Demonstrable experience managing Diabetes clinics and treatment plans
  • Experience supporting long-term conditions within general practice
