Key Responsibilities
- Lead and support diabetes reviews, monitoring, education, and treatment planning
- Undertake chronic disease management clinics and support long‐term condition pathways
- Complete patient assessments, care planning, and clinical interventions in line with practice protocols
- Provide health promotion, preventative care, and self‐management advice
- Administer vaccinations, routine treatments, and support medication reviews where appropriate
- Maintain accurate clinical records and documentation using the practice clinical system
- Work collaboratively with GPs, ANPs, HCAs, and wider multidisciplinary teams to support continuity of care
Requirements
- Professional registration with NMC or HCPC
- Demonstrable experience managing diabetes clinics and treatment plans
- Experience supporting long‐term conditions within a General Practice environment
