This role focuses on diabetes care and long‐term condition management, supporting high‐quality, patient‐centred care within a primary care setting.

Key Responsibilities

  • Lead and support diabetes reviews, monitoring, education, and treatment planning
  • Undertake chronic disease management clinics and support long‐term condition pathways
  • Complete patient assessments, care planning, and clinical interventions in line with practice protocols
  • Provide health promotion, preventative care, and self‐management advice
  • Administer vaccinations, routine treatments, and support medication reviews where appropriate
  • Maintain accurate clinical records and documentation using the practice clinical system
  • Work collaboratively with GPs, ANPs, HCAs, and wider multidisciplinary teams to support continuity of care

Requirements

  • Professional registration with NMC or HCPC
  • Demonstrable experience managing diabetes clinics and treatment plans
  • Experience supporting long‐term conditions within a General Practice environment
