Continuing Healthcare Nurse Assessor – Northamptonshire Location: Northamptonshire Contract Type: Temporary (Agency) Hours: Full-Time Pay Rate: £290/day (Umbrella) – Inside IR35 Service Care Solutions is currently recruiting for a Continuing Healthcare Nurse Assessor to join a dedicated team in Northamptonshire. This is an excellent opportunity for a skilled nurse to contribute to high-quality patient care and service delivery. About the Role: You will be responsible for: Facilitating multi-disciplinary assessments and patient reviews in line with national policies and procedures. Ensuring compliance with the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care. Supporting the Specialist Nurse/Clinical Manager in managing caseloads and delivering effective care. Promoting patient safety and personalised care planning, including Personal Health Budgets. Investigating eligibility for NHS Continuing Healthcare and managing disputes or challenges. Acting as Duty Clinician when required.What We’re Looking For: Experience in evaluating and measuring healthcare service performance. Ability to work collaboratively with clinicians, social care teams, and partner agencies. Skilled in conducting nursing assessments and interpreting complex evidence.
