Responsibilities: Conducting mental health assessments to understand patients’ needs, concerns, and risks. Developing personalized care plans with the patient, family, and multidisciplinary teams. Providing immediate support to patients in mental health crises, which could include prevention of hospital admission. Liaising with social workers, occupational therapists, and other health professionals to coordinate comprehensive care. Qualifications and Requirements: Fully enhanced valid DBS Mental Health Nursing degree required.
