Are you a compassionate and skilled Community District Nurse looking to make a real difference in patients lives? We are is seeking an experienced and motivated nurse to provide high-quality patient-centered care in the community.

Location: Guildford

Full-Time – 8AM to 8PM – 7 days

About the Role:

As a Community District Nurse, you’ll deliver essential healthcare services to patients in their homes, supporting their health and well-being while promoting independence. You’ll work collaboratively with an integrated care team to deliver personalized care that positively impacts the lives of individuals within the community.

Key Responsibilities:

  • Provide skilled nursing care to patients in their homes, including wound care, catheter management, medication administration, and more.
  • Assess, plan, implement, and evaluate patient care needs in collaboration with GPs and multidisciplinary teams.
  • Offer health education and emotional support to patients and their families.
  • Manage care delivery in a variety of community settings, ensuring patients receive the right care at the right time.
  • Maintain accurate patient records in accordance with care protocols and regulations.

What We’re Looking For:

  • Registered Nurse (RN) with valid NMC registration.
  • Experience in district or community nursing
  • Strong clinical skills and a patient-first approach to care.
  • Ability to work autonomously and manage a caseload effectively.
  • Excellent communication and interpersonal skills to engage with patients, families, and healthcare teams.
  • A full UK driving license and access to a vehicle is required
  • Non Medical Prescriber
  • DN SPQ Qualification
