Service Care Solutions are recruiting for an Adult Social Worker to join the Targeted Review Team within a local authority in Medway.
Key Responsibilities:
- Manage a mixed caseload within a community social work team
- Undertake strengths-based assessments and develop person-centred care plans
- Carry out face-to-face visits and interventions
- Support adults, families, and carers to promote independence and wellbeing
- Work in line with the Care Act, Mental Capacity Act, and safeguarding procedures
- Collaborate with multi-agency partners to deliver effective outcomes
- Maintain accurate case records and prepare professional reports
Requirements:
- Qualified Social Worker with Social Work England registration
- Completed ASYE
- Experience working with adults in a statutory setting
- Knowledge of relevant legislation (Care Act, MCA, etc.)
- Strong assessment and case management skills
