Location: York

Salary: Β£25,065 – Β£26,734 (Full-Time Equivalent)

Contract Type: Permanent – with opportunity for overtime with Bank Shifts

Hours: Full Time / Part Time / Flexible

About the Role

We are seeking motivated and organised Care Navigator to support our clients Clinical Services and Urgent Care teams. This role is central to ensuring patients receive high-quality care and a seamless experience. You will provide a wide range of administrative support and act as a key point of contact for both patients and healthcare professionals.

This post is non-patient facing.

Key Responsibilities

  • Deliver high-quality administrative support, including appointment booking and referral processing
  • Use SystmOne to manage patient data accurately and confidentially
  • Respond to patient and professional enquiries in a professional and compassionate manner
  • Support the coordination of care across services and follow up on patient needs
  • Work flexibly across teams to support service delivery

What We’re Looking For

Essential:

  • Level 2 English and Maths (or equivalent experience)
  • Excellent attention to detail and accuracy
  • Strong organisational and administrative skills
  • Excellent verbal and written communication
  • Confident using Microsoft Office applications
  • Ability to work independently and as part of a team
  • Experience in the NHS or general practice
  • Familiarity with SystmOne and referral systems such as Gateway
