About the Role
We are seeking motivated and organised Care Navigator to support our clients Clinical Services and Urgent Care teams. This role is central to ensuring patients receive high-quality care and a seamless experience. You will provide a wide range of administrative support and act as a key point of contact for both patients and healthcare professionals.

Key Responsibilities
- Deliver high-quality administrative support, including appointment booking and referral processing
- Use SystmOne to manage patient data accurately and confidentially
- Respond to patient and professional enquiries in a professional and compassionate manner
- Support the coordination of care across services and follow up on patient needs
- Work flexibly across teams to support service delivery
What Weβre Looking For
Essential:
- Level 2 English and Maths (or equivalent experience)
- Excellent attention to detail and accuracy
- Strong organisational and administrative skills
- Excellent verbal and written communication
- Confident using Microsoft Office applications
- Ability to work independently and as part of a team
- Experience in the NHS or general practice
- Familiarity with SystmOne and referral systems such as Gateway
