Location: Camden & Islington

Contract: Temporary – 3 Months Initially (With Likely Extension)

Hours: Monday – Friday 9:00AM – 5:00PM

Pay Rate: £23 P/H Umbrella Ltd (£20.59 P/H PAYE Inclusive/£18.37 P/H PAYE Exclusive)

The Opportunity:

Service Care Solutions are currently recruiting for a respected and well-established charity organisation delivering specialist substance misuse and homelessness support services across the community. The organisation is dedicated to supporting vulnerable individuals through compassionate, recovery-focused interventions and outreach support.

We are seeking an experienced Recovery Outreach Practitioner to support individuals experiencing substance misuse issues alongside homelessness or rough sleeping. The role will involve assertive street outreach, engagement and re-engagement work, delivering interventions and supporting individuals into treatment and recovery services.

Key Responsibilities:

  • Conducting street outreach to engage rough sleepers and vulnerable adults (Minimum 10,000 Steps Per Day)
  • Managing a small but complex caseload of service users
  • Completing assessments, risk assessments and recovery plans
  • Delivering one-to-one psychosocial interventions and harm reduction support
  • Supporting access to treatment, housing, healthcare and recovery services
  • Delivering needle exchange, naloxone and overdose prevention interventions
  • Working collaboratively with housing teams, local authorities and partner agencies
  • Maintaining accurate and timely case management records
  • Promoting safeguarding and responding appropriately to risk concerns

Essential Requirements:

  • Minimum 2 years’ experience within substance misuse services
  • Experience working with homeless, rough sleeping or socially excluded individuals
  • Strong understanding of safeguarding and risk management practices
  • Experience delivering outreach, engagement and recovery interventions
  • Ability to manage complex caseloads and maintain accurate case records
  • Excellent communication and relationship-building skills
  • Good IT skills including case management systems
  • NVQ Level 3 in Health & Social Care (or willingness to work towards)
  • Enhanced Adults & Children DBS (within 12 months or registered on Update Service)
  • Valid Right to Work documentation
  • Satisfactory employment references
