About the Role:
As a Social Worker - Adults Services, you will play a vital role in supporting adults within the Fylde & Wyre area. Your responsibilities will include coordinating hospital discharges, conducting assessments, and ensuring the best outcomes for service users. This position requires someone who is proactive, compassionate, and able to work independently.
Candidate Responsibilities:
- Conduct Continuing Health Care Assessments and Reviews.
- Complete Mental Capacity Assessments in accordance with the MCA 2005.
- Apply eligibility criteria under the Care Act effectively.
- Manage hospital discharge pathways to ensure timely and safe discharges.
- Triage for hospital discharges via SPA (Single Point of Access).
- Maintain accurate and detailed case records.
- Liaise with multidisciplinary teams, service users, and their families.
- Ensure compliance with relevant statutory and organizational policies.
Candidate Skills & Requirements:
- Ability to drive and access to personal vehicle is essential.
- Previous experience or knowledge of hospital discharge pathways.
- Strong understanding of the Care Act, MCA 2005, and adult social care processes.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage caseloads effectively.
- Immediate availability for interviews and start.
