Locum Adults Social Worker Home First Team Swansea City and County Council Location: Primarily Morriston Hospital with coverage at Singleton, Neath Port Talbot, and Gorseinon Swansea City and County Council is seeking to recruit a qualified and experienced Adults Social Worker to join the Home First Team on a locum basis. This is a fantastic opportunity for a dedicated social worker looking to make a meaningful impact by supporting adults during critical transition periods from hospital to home. The Home First Team plays a pivotal role in facilitating timely, safe and person-centred discharges from hospital settings back into the community. The team works collaboratively with health professionals, therapists, discharge coordinators and community partners to ensure individuals receive the right support at the right time. You will join a supportive, experienced group of professionals committed to strengths-based outcome-focused practice.
