Are you passionate about supporting children, young people, and families to achieve their full potential? We’re looking for an enthusiastic Family Hub Practitioner to join the Family Hubs team in the London Borough of Hammersmith & Fulham.
In this rewarding role, you’ll help deliver high-quality services within our Family Hubs network – supporting families from pregnancy through adolescence and helping improve outcomes in health, wellbeing, and education.
About the RoleAs a Family Hub Practitioner, you will:
- Support the delivery of activities across Family Hubs including Start for Life, After School and Youth Services.
- Provide a warm welcome to families through reception and sessional support.
- Assist with Family Hub outreach programmes across the borough.
- Work closely with multi-agency partners such as Health Visitors, Midwives, Youth Services, Schools, and Early Years providers.
- Offer guidance on baby and child development to help parents support their child’s growth and learning.
- Encourage families to engage with the Family Hub digital and virtual offer.
- Ensure the voice of the child and the family’s lived experience inform service design and delivery.
You’ll be based at various Family Hub sites in Hammersmith and Fulham, so flexibility and a proactive approach are key. Some evening or weekend work may be required to meet family needs.
About YouWe’re looking for someone who has:
- Experience in a customer-facing or family support role.
- Strong communication and organisational skills.
- A collaborative approach to working with partners and the community.
- A commitment to safeguarding and promoting the welfare of children and families.
- Competence in using Microsoft Office and maintaining accurate records.
Essential qualifications: GCSE English and Maths (Grade 4/C or above). An enhanced DBS check is required.
