We are currently recruiting for an experienced Adult Social Worker to join the Hospital Discharge Team on behalf of Hackney Council. This role plays a key part in supporting safe and timely hospital discharges, ensuring individuals receive the appropriate care and support when returning to the community.
You will work in a fast-paced, multidisciplinary hospital environment, collaborating with healthcare professionals, families, and community services to assess needs and coordinate effective discharge planning.
Key Responsibilities- Undertake strengths-based assessments to support safe hospital discharge
- Develop and implement person-centred care and support plans
- Work closely with NHS colleagues, discharge coordinators, and community services
- Facilitate timely discharges while ensuring appropriate care packages are in place
- Undertake safeguarding enquiries where required
- Apply legislation including the Care Act 2014 and Mental Capacity Act
- Maintain accurate case recordings and documentation in line with statutory requirements
- Qualified Social Worker with Social Work England registration
- Experience working within adult social care, ideally within a hospital discharge or acute setting
- Strong knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding procedures
- Ability to work effectively in a fast-paced multidisciplinary environment
- Excellent assessment, communication, and decision-making skills
- £32 per hour (Umbrella)
- 36 hours per week
