Care Co-ordinator Service care Solution are currently recruiting for a Care Co-ordinator in Enfield. The Care Co-ordinator will identify whether support packages continue to meet the needs of service users and their families. Pay rate – £16.30 PAYE / £19.93 Umbrella Main Responsibilities As a Care Co-ordinator you will be responsible for: To undertake initial assessments within the area of specialism. To manage, monitor and review care plans involving users and carers and liaising with other workers and agencies as appropriate. To be aware of the resources available within the specialist area, including other sources of funding so that the maximum benefit may be obtained for service users within budgetary resources of the Group. To make use of the management information systems, including information technology, in the recording retrieval and analysis of information as required by the department.
