Overview
Occupational Therapist – Reablement Service in Chippenham. Temporary ongoing contract with Service Care Solutions on behalf of a Local Authority.
Responsibilities
- Undertake Occupational Therapy assessments within the community and reablement settings.
- Support adults to maximise independence and achieve personal goals.
- Recommend appropriate equipment, adaptations, and moving and handling solutions.
- Develop and review reablement plans to promote safe and effective discharge from hospital and prevent admissions.
- Work closely with multidisciplinary teams, carers, and community services to deliver positive outcomes.
Qualifications and Skills
- Degree or Diploma in Occupational Therapy.
- HCPC Registration.
- Previous experience within Reablement, Community OT, Adult Social Care, or Intermediate Care services.
- Strong assessment and care planning skills.
- Full UK Driving Licence and access to a vehicle preferred.

- Rate of Pay: £36.00 per hour Umbrella; £28.78 per hour PAYE.
- Contract: Temporary Ongoing.
- Location: Chippenham, Local Authority client.
- Hours: Full-Time.
