Key Responsibilities
- Provide professional leadership and support within Adult Social Care services.
- Promote wellbeing, independence, and person-centred practice in line with the Care Act.
- Complete timely and robust assessments, reviews, and support plans.
- Safeguard vulnerable adults and manage complex safeguarding enquiries.
- Support individuals to remain independent within their communities for as long as possible.
- Work collaboratively with health professionals, families, carers, and partner agencies.
- Facilitate safe and timely hospital discharges and help prevent unnecessary admissions.
- Ensure services enhance existing informal support networks and community resources.
- Support people to achieve outcomes that are important to them while maintaining choice and control over their care and support.
Requirements
- Qualified Social Worker.
- Registered with Social Work England.
- Significant experience working within Adult Social Care.
- Strong knowledge of safeguarding legislation, policy, and practice.
- Experience managing complex cases and undertaking risk assessments.
- Excellent communication, decision-making, and leadership skills.
