Overview: Social Workers – Home First Hospital Team. 38.43 per hour, 37 hrs per week, 5 days office-based. Morriston with cover across local hospitals. 3-month contract (potential extension). Urgently recruiting 2 Qualified Social Workers to join a fast-paced Home First Hospital Team, supporting safe, timely discharges and coordinating community services across Morriston, Singleton, Neath Port Talbot and Gorseinon hospitals.
Responsibilities:
- Assess social care needs to enable safe discharge
- Coordinate community support packages
- Work closely with hospital teams, patients & families
- Manage caseloads and make timely, policy-led decisions
- Travel between hospital and community settings
Qualifications:
- Qualified Social Worker (UK registered)
- Hospital or community experience
- Full UK driving licence & own vehicle
- Strong communication and organisational skills
