Care Brokerage Officer

Lancashire ? Hybrid working - minimum 40% office-based ?

Full-time | Monday to Friday, with flexibility required

Do you have the ability to balance a person-centred approach with commercial awareness, problem-solving and the ability to find practical solutions?

We are currently recruiting for a Care Brokerage Officer to join a busy and important Adult Health & Wellbeing commissioning service.

This is a fantastic opportunity to play a key role in arranging and sourcing care and support for adults and young people preparing for adulthood, ensuring that services are person-centred, timely, effective and provide value for money .

The Role

As a Care Brokerage Officer, you will work closely with individuals, families, carers, providers and professionals to identify and arrange the most appropriate care and support options.

No two days will be the same.

You may be sourcing specialist placements, developing packages of care, negotiating with providers, exploring community-based support or helping someone access services that maximise their independence.

Your responsibilities will include

  • Identifying and arranging person-centred care and support packages.
  • Completing placement and provider searches across the care market.
  • Working with providers to source suitable, high-quality and cost-effective support.
  • Negotiating care packages and ensuring value for money.
  • Exploring free, low-cost and community-based services where appropriate.
  • Supporting a strengths-based and home-first approach to maximise independence.
  • Working across a range of service areas, including older people, health, mental health and learning disability services.
  • Maintaining accurate records and processing information through relevant case management systems.
  • Completing placement agreements, contractual documentation and service requisitions accurately and promptly.
  • Building strong relationships with providers, families, carers, service users and other professionals.
  • Responding to urgent and occasionally emergency care requirements, including same-day placements when required.

What We're Looking For

We are looking for someone who is confident, organised and solutions-focused, with the ability to manage a busy workload while maintaining a strong focus on the individual.

You should have

  • Experience within Adult Social Care, care brokerage, commissioning, care coordination or a similar environment.
  • Experience of sourcing placements or developing packages of care.
  • Strong communication and negotiation skills.
  • The ability to manage challenging conversations and find practical solutions.
  • Experience working collaboratively with providers and other agencies.
  • Excellent attention to detail and the ability to work accurately in a fast-paced environment.
  • Experience maintaining records containing sensitive information.
  • Confidence using ICT and case management systems.
  • A good understanding of the legislation and regulatory framework surrounding Adult Social Care.
  • Knowledge or experience of personal budgets and Direct Payments would be highly beneficial.
  • An administrative or business management qualification at NVQ Level 3 or above, or a willingness to work towards this.

You'll be helping to ensure people receive the right support, in the right place, at the right time , while promoting independence, choice and better outcomes.

You'll need to be commercially aware, organised and confident communicating with a wide range of people - but most importantly, you'll understand that behind every referral or placement request is a person whose life can be positively impacted by getting the right support in place.

The role offers hybrid working, although employees are expected to spend a minimum of 40% of their working time on-site.

Flexibility is also required, including occasional home visits and the ability to respond to urgent service demands when necessary.
