Are you an experienced and dedicated Social Worker looking for an exciting contract opportunity?
We are currently seeking a skilled Social Worker to join our team for a two-week assignment to support our carers review process.

About the
Role: As a Social Worker, you will be responsible for reviewing carers’ breaks in compliance with the Care Act, ensuring support arrangements are right-sized to meet assessed needs, and facilitating the transition from voucher-based systems to Direct Payments.
Your focus will also include empowering carers through personalised, outcome-focused support planning, ensuring their needs are adequately met while promoting independence.
Responsibilities
- Conduct high-quality Care Act-compliant Carers Reviews and assessments
- Develop and implement person-centred, proportionate, and legally compliant support plans
- Manage sensitive conversations regarding changes or reductions in services
- Align support services to individual needs, making evidence-based, defensible recommendations
- Transition carers to Direct Payments, working closely with Direct Payments and Brokerage Teams
- Ensure care packages deliver value for money within budget and policy frameworks
- Maintain accurate and timely case records on the care management database
- Demonstrate excellent assessment and analytical skills
- Manage a high-volume caseload efficiently and professionally
- Make evidence-based decisions to promote best outcomes for carers and service users
Qualifications
- Registered with Social Work England (SWE)
- Experienced in Care Act assessments and reviews
- Strong communication, negotiation, and analytical skills
- Ability to handle sensitive conversations with empathy and professionalism
- Skilled in resource and financial management within support settings
