Service Care Solutions are working alongside a leading healthcare provider in Leeds who are looking to recruit two ACP’s to join their team on a permanent basis.
Position Details
- Role: 2x Advanced Clinical Practitioner
- Location: Leeds
- Working: On-site (flexible shifts across GP Surgeries and Urgent Care Services)
- Hours: Full-time (pro-rate opportunities also)
- Salary: £61,984 to £80,828 per year (based on a 40-hour week)
- Contract: Permanent
Job Purpose
- Qualififed ANP, ACP or ENP. Previous experience working within minor injuries, urgent care or an equivalent acute clinical setting.
- Work independently as an Advanced Clinical Practitioner (ACP) across primary and urgent care environments, assessing and managing patients of all ages and with a wide range of presenting conditions.
- Independently prioritise, assess, diagnose, treat, refer and safely discharge patients with undifferentiated and undiagnosed primary and urgent care presentations.
- Deliver consultations through appropriate modalities, including face-to-face, telephone and, where required, home visits, working within agreed clinical competence and service requirements.
- Act as a referral agent and care coordinator, working collaboratively across primary, secondary and community services to ensure patients receive appropriate, person-centred care.
- Prescribe safely and appropriately within current legislation, professional scope of practice and approved Patient Group Directions.
- Provide clinical leadership, advice, mentorship and professional support to colleagues, including trainee ACPs, UCPs and GP Registrars.
- Maintain accurate, contemporaneous clinical records and ensure all care is delivered in accordance with relevant professional standards, policies, guidelines and protocols.
- Promote evidence-based practice and contribute to clinical audit, data collection, activity monitoring, quality improvement and the development and review of clinical protocols.
- Recognise, report and actively manage patient safety incidents, supporting organisational learning and contributing to investigations in line with Just Culture principles.
- Promote integrated and cross-functional working with healthcare professionals, services and external agencies, supporting effective communication and coordinated patient care.
- Maintain the highest standards of patient safety, quality and safeguarding, including recognising and appropriately responding to concerns involving children and vulnerable adults.. Maintain professional registration, competence and revalidation requirements through continuing professional development, clinical supervision, mandatory training and the Personal Development Review process.
- Comply with organisational requirements relating to information governance, confidentiality, infection prevention and control, corporate governance, financial procedures and Freedom to Speak Up.
