Service Care Solutions is recruiting an experienced ANP or ACP to support a GP Surgery on a locum basis. The role is suited to a nurse seeking full‑time hours, with flexible availability across Monday to Friday also considered.

Contract: Long‑Term Locum
Hours: Up to 37.5 hrs per week – Part‑time considered
Pay: £50 LTD p.h. / £44 PAYE inclusive p.h.
This role has a strong focus on diabetes care and long‑term condition management, supporting the delivery of high‑quality, patient‑centred care within a primary care setting.
Key Responsibilities:
- Lead and support diabetes reviews, monitoring, education, and treatment planning.
- Undertake chronic disease management clinics and support long‑term condition pathways.
- Complete patient assessments, care planning, and clinical interventions in line with practice protocols.
- Provide health promotion, preventative care, and self‑management advice.
- Administer vaccinations, routine treatments, and support medication reviews where appropriate.
- Maintain accurate clinical records and documentation using the practice clinical system.
- Work collaboratively with GPs, ANPs, HCAs, and wider multidisciplinary teams to support continuity of care.

- Professional registration with the NMC or HCPC.
