About the Role
As an Adult North Team - Adult SW, you will work within a small, dynamic team based in North.
You will conduct Care Act assessments, reviews, CHC checklists, Discharge to Assess (DTA), and Care of the Person (COP) work as needed.
The role involves participating in a duty rota to ensure client needs are met efficiently and compassionately.
Responsibilities
- Conduct Care Act assessments, reviews, and relevant assessments
- Complete CHC checklists, DSTs, and COP work where appropriate
- Participate in a duty rota covering office and community visits
- Work collaboratively with team members to ensure high standards of social care
- Maintain accurate and up-to-date case records and documentation
- Comply with all relevant safeguarding policies and procedures
Qualifications
- Proven experience as a Social Worker, preferably within adult social care
- HCPC registration is mandatory
- Experience in completing assessments, reviews, and care planning
- Ability to work effectively as part of a team and independently
- Flexibility to work in a hybrid setting (minimum 2 days in the office)
- Comprehensive understanding of the Care Act and related legislation
