About the Role
This role involves working within a Wellbeing Hub, a flexible, drop-in service offering access to a range of specialist support including:
- Symptom management
- Advice and outpatient clinics
- Wellbeing and rehabilitation sessions
The Hub is designed to support individuals living with life-limiting illnesses, as well as their families and carers. The focus is on enhancing wellbeing, promoting independence, and helping people live as well as possible with their condition.
Key Objectives
- Promote and model high-quality patient care while supporting junior staff.
- Assist in the delivery of Specialist Palliative Care to patients and their families.
- Maintain high standards of care at all times.
- Provide compassionate, safe, and dignified care across a range of settings.
- Welcome and orient patients and families to the care environment.
- Support the delivery and monitoring of evidence-based nursing care under the supervision of a registered nurse.
Person Specification
- 2 Years minimum UK based experience within a clinical environment, including Mental Health, Palliative Care.
- Exceptional written and verbal communication
- NVQ Level 2 in Health and Social Care
- Full UK driving licence.
