Are you an experienced Care Navigator looking for a fulfilling contract opportunity? We are seeking a dedicated Care Navigator to provide vital support across GP Practices and our client Care Hubs in Sefton and the wider Liverpool area. This Care Navigator role offers a chance to make a real difference in patients’ lives while working flexible hours on an ad hoc basis.
Responsibilities
- Providing administrative support to GP Practices and our client Care Hubs
- Handling patient queries and offering information on services and procedures
- Working closely with healthcare professionals to ensure seamless patient care
- Maintaining accurate records and databases using EMIS
- Offering patient navigation duties and supporting patients throughout their journey
This Care Navigator role is ideal for individuals with previous experience as a Receptionist or Care Navigator within a GP or health centre. Your expertise will ensure patients receive the highest quality support and care.
