Tameside Adult Social Care is seeking an experienced and passionate Mental Health Transitions Social Worker to join our specialist team supporting young people as they move from Children's Services into Adult Services. This is a rewarding opportunity to work with young people experiencing moderate to severe mental health difficulties, including those with autism and dual diagnoses, ensuring they receive the right support at a critical stage in their lives.

Key Responsibilities

  • Support young people transitioning from Children's Services into Adult Services.
  • Undertake Care Act assessments and develop outcome‑focused support plans.
  • Complete funding applications and statutory documentation.
  • Manage complex legal work including Court of Protection DoLS applications, DoLS, Section 21A challenges and court reports.
  • Build meaningful engagement with young people who may be difficult to engage and support their families and carers.
  • Undertake Carer's Assessments and support carers under the Care Act.
  • Work closely with SEND services and understand the role of Education, Health and Care Plans (EHCPs).
  • Promote strengths‑based practice and person‑centred planning.
  • Collaborate across health, education, children's services and adult social care to ensure seamless transitions.
  • Contribute to a responsive six‑day service, primarily Monday to Friday with occasional Saturday working.

About You

We are looking for an experienced and motivated Social Worker who is passionate about improving outcomes for young people and their families.

Qualifications

  • Social Work Degree, DipSW or equivalent professional Social Work qualification.
  • Completion of ASYE.
  • Current registration with Social Work England.
  • Full UK driving licence and access to a vehicle for work purposes.

Experience

  • Minimum of 2 years post‑qualification experience.
  • Substantial experience working with young people transitioning from Children's Services into Adult Services.
  • Significant experience working within Mental Health Services.
  • Knowledge and experience of SEND and Education, Health and Care Plans (EHCPs).
  • Experience of Care Act assessments, care planning, safeguarding and statutory social work practice.

Salary & Benefits

  • Salary Grade H (SCP 29‑34):
    • SCP 29 – £39,862
    • SCP 30 – £40,777
    • SCP 31 – £41,771
    • SCP 32 – £42,839
    • SCP 33 – £44,075
    • SCP 34 – £45,091
  • Annual Leave Grade G‑N:
    • Up to 10 years’ service: 28 days annual leave
    • 10 years’ service: 31 days annual leave

AMHP Opportunities

AMHP qualification is welcomed but not essential. Qualified and warranted AMHPs receive an additional payment of £82 per AMHP shift (approximately 25‑30 shifts available annually). For candidates who are not currently AMHP qualified, Tameside is committed to supporting professional development and can provide opportunities to undertake AMHP training in the future.

Working Pattern

Full‑time hours: Monday: 8:30am – 5:00pm, Tuesday: 8:30am – 5:00pm, Wednesday: 8:30am – 5:00pm, Thursday: 8:30am – 4:30pm, Friday: 8:30am – 4:00pm. The service operates across six days, with occasional Saturday working where required. The team is currently working towards offering one working‑from‑home day per week, agreed with managers.
