Job Description

Senior Service Co-ordinator – Primary CareLocation: YorkContract: Permanent – 37.5 Hours Per WeekWe are seeking a highly organised and proactive Senior Service Co-ordinator to join our Primary Care Team.This pivotal role ensures the smooth running of services across our primary care service, supporting the delivery of safe, efficient, and patient-centred care. Working closely with GPs, practice managers, and a range of healthcare professionals, you will help shape services that meet the needs of our local community.This is an excellent opportunity for an experienced healthcare administrator, service manager, or co-ordinator who thrives in a fast-paced environment and is passionate about making a difference.
  • Oversee and co-ordinate day-to-day service delivery across practices within the service.
  • Line-manage and support administrative and co-ordination staff to ensure smooth service operation.
  • Develop and maintain effective systems for patient flow, referrals, scheduling, and reporting.
  • Act as a central point of contact for staff, patients, and partner organisations.
  • Support delivery of national and local initiatives, such as enhanced access, frailty pathways, vaccination programmes, and long-term condition management.
  • Monitor service performance, analyse data, and prepare reports for the leadership team.
  • Identify and implement improvements to enhance patient experience and operational efficiency.
