Job Description
- Provide senior clinical leadership and decision-making within the Virtual Frailty Ward.
- Undertake assessments of acutely unwell or deteriorating frail patients.
- Develop and implement holistic, person-centred care plans, including advanced care planning where appropriate.
- Liaise closely with consultants, ACPs, community nurses, therapists, pharmacists, and social care professionals.
- Prescribe, monitor, and review medications in line with best practice and patient needs.
- Contribute to service evaluation, quality improvement initiatives, and education for colleagues and trainees.
- Participate in rota cover, including virtual ward rounds and urgent clinical reviews.
