Team Base: One Care Home Team, Barnet Integrated Care Partnership

Job Purpose: We are currently recruiting a compassionate and experienced Band 6 Registered General Nurse (RGN) to join the One Care Home Team based at Edgware Community Hospital. This long-term contract offers a unique opportunity to make a significant difference to the lives of residents in care homes across Barnet. As part of a proactive and dynamic multi-disciplinary team, you will provide both reactive and proactive face-to-face and virtual clinical support to residential and nursing home residents. This role is key in the delivery of high-quality, person-centred care, supporting complex healthcare needs and ensuring seamless, integrated care planning across services.

Key Responsibilities:

  • Caseload Management: Manage and coordinate a dedicated caseload of care home residents within the Barnet area.
  • Clinical Assessments: Deliver comprehensive clinical assessments and contribute to MDT planning and reviews.
  • Care Planning: Develop, implement, and evaluate personalized care plans in collaboration with care home teams and other stakeholders.
  • Interdisciplinary Collaboration: Work closely with GPs, therapists, social care staff, and other MDT members to ensure continuity and quality of care.
  • Patient Advocacy: Champion access to healthcare for residents facing barriers, promoting equality and dignity in care.
  • Communication & Documentation: Maintain effective communication networks and accurate clinical records in line with professional and legal standards.
  • Leadership & Mentorship: Provide leadership in clinical practice and contribute to the education and support of peers, students, and care staff.
  • Professionalism: Demonstrate high standards of clinical care, conduct, and accountability at all times.

Requirements:

  • NMC Registration as a Registered General Nurse (RGN) – Band 6 level
  • Previous experience in community nursing, care homes, or adult complex care
  • Strong assessment, care planning, and communication skills
  • Ability to work autonomously and as part of an MDT
  • Flexibility to travel to care homes across the Barnet area
