Remote/Hybrid Working Nurse Manager – Community Complex Care
Salary: £48,000

Sentra Care are working in partnership with our client, a recognised leader in the Community Complex Care sector, to recruit an experienced Clinical Lead to support adults and children with complex healthcare needs in the community. This is a highly autonomous, clinically focused role offering a true hybrid working model, with the opportunity to make a meaningful impact on client outcomes while maintaining a healthy work–life balance.
You will be responsible for managing a community-based caseload across Basingstoke, Reading and Oxford, ensuring the delivery of safe, high-quality, nurse-led care:
- 3 days per week in the community, completing client assessments, staff training, touchpoints, reviews, and clinical oversight
- 1 day per week working from home for care planning, governance, and MDT coordination
- Office Days for team meetings and training
You will work closely with Care Coordinators, operational teams, and senior clinical leadership to ensure robust, client-centred care delivery. You will report into the Regional Lead Nurse and work alongside Practice Educators and Care Coordinators.
Key Responsibilities:
- Build strong relationships with clients, families, community teams, and specialist clinicians
- Work with central resourcing teams to ensure a skilled and consistent care workforce
- Create, review, and update person-centred care plans and risk assessments
- Provide visible, hands-on clinical support in the community
- Participate in the nurse on-call rota
- Escalate clinical and safeguarding concerns appropriately
- Support complaint resolution and clinical investigations
- Ensure compliance with clinical policies and regulatory standards
Requirements:
- NMC Registered Nurse
- Background in ICU/ITU/HDU or complex continuing healthcare - MUST HAVE
- Recent Tracheostomy & Ventilation experience - within last 6 months
- Right to Work in UK - They cannot offer Skilled worker visas at this stage
- Post-qualification experience in acute clinical care
- Ability to work autonomously and manage a community caseload
- Full UK driving licence and access to your own vehicle
