Sentra Care has partnered with a leading community Complex Care provider to recruit a Critical Care Nurse to join their growing clinical team. This is an exciting opportunity for an experienced nurse looking to move away from a traditional hospital environment whilst continuing to utilise their specialist clinical skills to support individuals with highly complex needs within the community. The organisation supports clients living with a range of complex physical and neurological conditions, including tracheostomy and ventilator‑dependent individuals, as well as those with acquired brain injuries, spinal cord injuries, and other life‑limiting conditions. The successful candidate will play a key role in ensuring safe, effective, and person‑centred care is delivered across a community caseload. This is a remote/hybrid role, combining community‑based visits, home working, and one day per week in the London office.

The Role

As a Critical Care Nurse, you will oversee a caseload of clients living in the community, providing clinical oversight, conducting assessments, supporting care teams, and ensuring the highest standards of care delivery, quality, and governance. You will work closely with clients, families, support workers, healthcare professionals, and internal stakeholders to ensure care packages remain safe, effective, and responsive to changing needs.

Key Responsibilities

  • Create, review, and update person‑centred care plans and clinical risk assessments
  • Conduct comprehensive client assessments and reviews within the community
  • Provide visible, hands‑on clinical support to clients and care teams
  • Undertake client reviews at least every 8 weeks, ensuring care remains appropriate and effective
  • Support the implementation and monitoring of complex clinical interventions
  • Participate in the Nurse On‑Call rota
  • Escalate clinical, safeguarding, and risk concerns appropriately
  • Support complaint management, incident investigations, and clinical reviews
  • Ensure compliance with internal clinical policies, CQC requirements, and regulatory standards
  • Deliver clinical supervision, mentoring, and support to care staff
  • Contribute to continuous quality improvement initiatives across the service

About You

You will be an experienced nurse with a strong critical care background and recent hands‑on experience supporting individuals with tracheostomies and ventilation requirements. You will be comfortable working autonomously, managing a community caseload, and building strong relationships with clients, families, and multidisciplinary teams.

  • Registered Nurse (NMC)
  • Background within ICU, ITU, HDU, Critical Care, or Complex Continuing Healthcare
  • Recent tracheostomy and ventilation experience within the last 6 months (essential)
  • Post‑registration experience within an acute clinical setting
  • Experience mentoring, supervising, and developing staff
  • Strong communication, organisational, and analytical skills
  • Ability to work independently and effectively manage a community caseload
  • Full Right to Work in the UK (please note sponsorship is not available for this position)
  • Must be based within North London or surrounding areas and able to travel throughout the region
  • Must have a UK Driver Licence & Own Car
