Rehabilitation Case Manager (Occupational Therapist) London | Hybrid (Remote & Community Based)
Salary: 40,000 - 42,000 + 4,200 Car Allowance
Bonus: 12% of annual salary (paid quarterly)
Hours: Full-time, 37.5 hours per week - Part time options also available
Sentra Care is partnering with one of the UKβs largest and most reputable Case Management companies to recruit a Rehabilitation Case Manager. This is an exciting opportunity for an Occupational Therapist looking to move into a role offering true autonomy, where you can directly improve the lives of the clients you work with while managing your own caseload and diary.
The role is remote-based, covering clients across London. A typical week will involve three days working from home focused on case management and report writing, and two days out in the community completing assessments and attending meetings.
The Role
- Delivering high-quality, outcome-focused rehabilitation by:
- Completing Initial Needs Assessments
- Managing a caseload of clients across Brain injury, Spinal Injury, Trauma & Orthopaedics
- Identifying barriers to recovery and developing tailored rehabilitation programmes
- Coordinating treatment and liaising with clinicians and service providers
- Monitoring client progress through face-to-face, written, phone, and video contact
- Producing clear, timely reports and maintaining accurate case records
- Arranging or undertaking specialist assessments where required
- Building strong professional relationships with referrers and key stakeholders

What We're Looking For
- Degree in Occupational Therapy
- HCPC registration
- Strong communication, clinical reasoning, and problem-solving skills

- Must have right to work in the UK
